Wheeler Health Office

Physical examinations are required for all children new to the Onondaga Central School District and students entering the 3rd and 5th grades at Wheeler School.  It is required that proof of a current physical exam be submitted to the Health Office within 30 days of the commencement of the school year. Physical exams are valid for a 12 month period.  Physical examinations are also required for students entering 7th grade.

School physical exams will be offered in the fall.   If you would like to schedule a school physical for your child, please call the school nurse Lindsey Yonko at 315-552-5052.  
 



Health Office Information and Forms

Our Health Services Staff provides a wide range of health support to our district's children including:
First aid and illness care during the school day
Administration of medications and treatments
Development of Emergency and Individualized Healthcare Plans as needed for students with significant health issues. 
Health screenings:
Distance and near visual acuity for all new entrants and for children in grades Pre-K or Kindergarten, 1, 3, 5, 7, and 11 or at any other time deemed essential.
Color perception for all new entrants within six months of the entrance.
Hearing acuity for all new entrants and for those in grades Pre-K or Kindergarten, 1, 3, 5, 7, 11, and at any other time deemed essential.
Scoliosis (curvature of the spine) for female students in grades 5 and 7 and for male students in grade 9.
Provide health-related education to students, parents, co-workers, and the community based on health-related issues that arise on an individual, classroom, building or community level.
Forms are available for parent/guardian use (see below).  Physical forms must be on file in the Health Office for all new entrants as well as students in Pre-K, Kindergarten, Grades 1, 3, 5, 7, 9, and 11 within 30 days of the commencement of the school year.  If a student participates in sports, he/she must have an annual physical.  Medication guidelines for parents/guardians are also available below.  Please print, complete, and return these forms to your student's Health Office.

New York State requires all districts to participate in surveys directed by the state commissioner of health.  The student's identity will not be disclosed by the health department.  For further information, click here.
