Information and Forms
Our Health Services Staff provides a wide range of health support to our district's children including:
- First aid and illness care during the school day
- Administration of medications and treatments
- Development of Emergency and Individualized Health Care Plans as needed for students with significant health issues.
- Health screenings:
- Distance and near visual acuity for all new entrants and for children in grades Pre-K/Kindergarten, 1, 2, 3 5, 7, and 11, or at any other time deemed essential.
- Color perception for all new entrants within six months of entrance.
- Hearing acuity for all new entrants and for those in grades pre-k/kindergarten, 1, 3, 5, 7, 11, and at any other time deemed essential.
- Scoliosis (curvature of the spine) for girls in grades 5 and 7 and all boys in grade 9.
- Sports qualification for all competing athletes.
- Provide health related education to students, parents, co-workers and the community based on health related issues that arise on an individual, classroom, building or community level.
New York State requires all districts to participate in surveys directed by the state commissioner of health. Student's identity will not be disclosed by the health department.

School/Sports Physicals
School/sports physicals are offered twice each year to students: once each fall and then again in the spring. School physicals will be offered on Wednesday, October 25, 2023; they will not be offered again until May, 2024. Forms are available for parent/guardian use (see below). Physical forms must be on file in the Health Office for all new entrants as well as students in Pre-K, Kindergarten, Grades 1, 3, 5, 7, 9 and 11 within 30 days of the commencement of the school year. If a student participates in sports, he/she must have an annual physical. Medication guidelines for parents/guardians are also available below. Please print, complete, and return these forms to your student's Health Office.
